These issues and fixes address a bug where the kanban check_respawn_guard incorrectly triggers blocker_auth when spawn or workspace errors happen to contain auth/quota-related slug words, causing tasks to become permanently wedged in the 'ready' state. The fixes scope blocker_auth detection to actual run outcomes rather than raw error string matching.
